At the Y, we believe that true wellness extends beyond our walls. Community Integrated Health connects healthcare providers, community partners, and Y programs to bridge the gap between clinical care and everyday healthy living. Our goal is simple: to ensure every individual, regardless of age, ability, or health status has access to the support, education, and resources needed to live a healthier, more vibrant life.
Through this initiative, we create seamless pathways for participants transitioning from medical treatment or chronic condition management into safe, supportive fitness and wellness environments. From cardiac rehab graduates and cancer survivors to those managing arthritis, Parkinson’s, diabetes, or multiple sclerosis, our specialized programs help people stay active, engaged, and connected to their community.
Our certified wellness professionals, personal trainers, and registered nurses provide individualized guidance, ensuring that participants exercise safely and confidently. Each program emphasizes education, accountability, and measurable progress, empowering members to take control of their health while preventing future complications.
Community Integrated Health is more than just fitness; it’s about connection. It’s about creating partnerships with local hospitals, physicians, and health networks so that together we can improve population health, reduce barriers to care, and make wellness accessible to all.
Join us in building a community where healthcare and healthy living work hand in hand, because lasting health begins where we live, work, and play.
